Volunteer Acknowledgment The Volunteer acknowledges and agrees that participation in the activities of Made to Serve is voluntary and may involve risks, including but not limited to, personal injury, property damage, and illness. By signing this Waiver, the Volunteer acknowledges that they understand the risks involved and assume full responsibility for any injury or loss resulting from participation. 2. Waiver and Release The Volunteer, for themselves and their heirs, personal representatives, and assigns, hereby voluntarily and irrevocably releases, waives, discharges, and holds harmless Made to Serve from any and all claims, demands, causes of action, liabilities, damages, or losses, whether known or unknown, arising out of or in connection with the Volunteer’s participation in any activities with Made to Serve, including but not limited to, the preparation and distribution of meals to the homeless. 3. Medical Treatment The Volunteer understands and agrees that Made to Serve does not assume any responsibility for or obligation to provide financial assistance, medical care, or insurance in the event of injury or illness. The Volunteer agrees to be solely responsible for any costs related to medical treatment or other damages in connection with participation in the activities of Made to Serve. 4. Indemnification The Volunteer agrees to indemnify and hold harmless Made to Serve from any claims, lawsuits, or liabilities, including reasonable attorney’s fees and costs, that may arise from any negligent or wrongful acts by the Volunteer while participating in the organization’s activities. 5. Photographic Release The Volunteer grants and conveys to Made to Serve all rights, title, and interest in any and all photographs, images, video, or audio recordings of the Volunteer made during the Volunteer’s participation with Made to Serve, including, but not limited to, any royalties, proceeds, or other benefits derived from such recordings. 6. Volunteer Conduct The Volunteer agrees to adhere to the policies, procedures, and guidelines set forth by Made to Serve and understands that failure to comply with such may result in dismissal from volunteer activities. 7. Governing Law This Waiver shall be governed by and interpreted in accordance with the laws of the state in which Made to Serve operates, without regard to its conflict of law principles. 8. Severability In the event that any provision of this Waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
